2022 COVID Compliance Form for Summer Camps and Programs

I am the parent or legal guardian of __________________________________, a minor child, who is participating in the [NAME OF CAMP OR PROGRAM] at the University of Delaware.  As a condition of my child’s participation, I confirm that I understand and agree on behalf of myself and my child, that I am responsible for ensuring my child’s compliance with the following:

1. Before my child arrives on the first day of the camp or program, I understand that my child must obtain a negative COVID test within 24 hours of arrival to camp. (Lab-based PCR/NAAT/Antigen tests and home antigen tests are permitted).  This test result does not need to be submitted to UD or the Camp or Program, in which my child is participating, but I am representing that it will be done.  
2. My child will follow UD’s current policies on masking and other COVID protocols at all times.
3. My child will be screened for symptoms at home and remain at home should any symptoms arise. 
4. My child will follow guidance provided by the Delaware Public Health Department, Center for Disease Control and my own health care providers regarding at-home isolation protocols.
5. If my child received a positive COVID test, I understand that I must share that information with the Summer Camp or Program Director in order to assess if any communications or actions need to take place.
